GLP-1 Side Effects: What to Expect and How to Manage Them

Understanding GLP-1 Side Effects

GLP-1 receptor agonists like semaglutide and tirzepatide are among the most effective weight-loss treatments available, but they do come with side effects — particularly during the early weeks of treatment. The good news: most side effects are mild-to-moderate, temporary, and highly manageable with the right strategies.

Most Common Side Effects

Nausea (40-44% of patients)

Nausea is the most frequently reported side effect, particularly during dose escalation. It usually peaks in the first few days after an injection and improves over time as your body adjusts. Most patients report nausea largely resolving within 4-8 weeks.

How to manage it:

  • Eat smaller portions and avoid high-fat, greasy foods
  • Eat slowly and stop when you feel full
  • Stay upright for 30-60 minutes after eating
  • Ginger tea, ginger chews, or peppermint can help
  • If severe, your provider can prescribe anti-nausea medication

Vomiting (15-24% of patients)

Vomiting is less common than nausea and typically occurs when nausea is not well-managed. Eating too much or too fast is a common trigger. Take small, frequent sips of water to stay hydrated and stick to bland foods like crackers, toast, or rice when symptoms are active.

Diarrhea (30% of patients)

GLP-1 medications speed up intestinal motility in some people. Stay well hydrated with electrolyte-rich fluids and avoid high-fat or spicy foods during flares. OTC loperamide (Imodium) can be used for short-term relief.

Constipation (24-30% of patients)

Paradoxically, some patients experience constipation because GLP-1 medications slow gastric emptying. Increase daily water intake significantly, add soluble fiber, and use MiraLax or fiber supplements as needed.

Less Common but Important Side Effects

Injection Site Reactions

Redness, itching, or small lumps at the injection site occur in some patients. Rotating injection sites each week reduces this risk.

Heartburn / GERD

Because GLP-1 medications slow gastric emptying, some patients experience increased reflux. Eating smaller meals and avoiding eating close to bedtime helps.

Rare but Serious Side Effects

Pancreatitis

Acute pancreatitis has been reported rarely with GLP-1 medications. Symptoms include sudden, severe abdominal pain radiating to the back, nausea, and vomiting. Seek emergency care immediately if you experience this.

Thyroid C-Cell Tumors

Animal studies showed increased thyroid tumors at very high doses. As a precaution, GLP-1 medications are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2.

The Dose Escalation Strategy

GLP-1 medications are deliberately started at a low dose and slowly increased over months. This gradual escalation is the single most effective strategy for minimizing side effects. Always follow your provider's titration schedule.

When to Contact Your Provider

Reach out if you experience severe or persistent vomiting or diarrhea, signs of dehydration, sudden severe abdominal pain, resting heart rate increase greater than 15 bpm, or vision changes.
