Semaglutide vs Tirzepatide: Which GLP-1 Is Right for You?
The Two Leading GLP-1 Weight Loss Medications
If you have been researching medical weight loss, you have almost certainly come across two names: semaglutide and tirzepatide. Both are injectable medications that suppress appetite and promote significant weight loss. But there are meaningful differences — and understanding them can help you and your provider make the best choice for your body and health goals.
How They Work
Semaglutide: Single GLP-1 Agonist
Semaglutide (brand names Ozempic® for diabetes, Wegovy® for weight loss) works by activating GLP-1 receptors only. GLP-1 is a hormone released after eating that signals the brain to reduce appetite, slows gastric emptying, and helps regulate blood sugar. By mimicking this hormone, semaglutide reduces hunger and caloric intake.
Tirzepatide: Dual GIP/GLP-1 Agonist
Tirzepatide (brand names Mounjaro® for diabetes, Zepbound® for weight loss) is a "twincretin" — it activates both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors simultaneously. GIP is another gut hormone involved in fat metabolism and energy balance. This dual action may explain why tirzepatide produces greater weight loss than semaglutide in direct comparisons.
Clinical Trial Results
The STEP trials for semaglutide showed average weight loss of 14.9-16.9% over 68 weeks. The SURMOUNT-1 trial for tirzepatide (published in NEJM, 2022) showed participants on 15 mg tirzepatide lost an average of 22.5% of body weight — results that were unprecedented for a non-surgical treatment. At the highest dose, 56% of participants lost at least 20% of their body weight.
Side Effects: Are They Different?
Both medications share similar GI side effects, particularly during dose escalation: nausea, vomiting, diarrhea or constipation, and stomach discomfort. Some studies suggest tirzepatide's GI side effects may be slightly milder, possibly because GIP receptor activation has a protective effect on the gut. However, individual responses vary significantly.
Which Should You Choose?
Consider Semaglutide If:
- You want a well-established medication with the longest real-world track record
- You have tried tirzepatide and had significant side effects
- Cost or availability is a primary concern in your area
- You have diabetes and your provider recommends it for blood sugar control
Consider Tirzepatide If:
- You want the medication with the highest average weight loss in clinical trials
- You have tried semaglutide and hit a plateau
- Your BMI is higher and you are aiming for more significant total weight loss
- Your provider recommends it based on your metabolic profile
The Bottom Line
Both medications are dramatically more effective than any previous non-surgical weight loss treatment. Tirzepatide produces greater average weight loss, but semaglutide is highly effective for most people and has a longer track record.
