How to Self-Inject GLP-1 Medication: A Step-by-Step Guide

Self-injection is simple and nearly painless with the right technique. Follow these steps for a safe, effective injection every time.

Step-by-Step Instructions

Gather Your Supplies

  • Your pre-filled syringe or auto-injector pen (provided in your kit)
  • Alcohol swabs (included in your kit)
  • A sharps disposal container
  • A clean, well-lit surface to work on

Tip: Remove your medication from the refrigerator 30 minutes before injecting. Room-temperature medication is more comfortable.

Choose Your Injection Site

  • Abdomen (belly): at least 2 inches away from your navel — most common site
  • Upper thigh: outer, middle portion of either thigh
  • Upper arm: outer portion of the upper arm (easier with help)

Tip: Rotate injection sites each week to prevent skin irritation or lipodystrophy (lumpy skin). Keep a simple log of where you injected last.

Clean the Area

  • Wash your hands thoroughly with soap and water for at least 20 seconds
  • Open an alcohol swab and wipe the injection site in a circular motion
  • Allow the skin to dry completely — 10–15 seconds — before injecting

Tip: Do not blow on the area or wave your hand to dry it faster. Let it air dry naturally.

Prepare the Pen or Syringe

  • Check the medication: it should be clear and colorless (or very slightly yellow). Do not use if cloudy or contains particles.
  • Remove the cap from the pen or needle. Do not touch the needle.
  • If using a pen with a dose dial, set it to your prescribed dose.
  • Prime the pen if instructed (your provider instructions will specify).

Tip: Always check the expiration date on your medication before use. Keep unused medication in the refrigerator.

Inject the Medication

  • Gently pinch about 1–2 inches of skin at your chosen site
  • Insert the needle at a 90° angle (or 45° if using a longer needle with less body fat)
  • Press the plunger or button slowly and steadily until the full dose is delivered
  • Hold for 5–10 seconds before removing to ensure full delivery
  • Release the pinch and withdraw the needle smoothly

Tip: Do not rub the injection site after — this can cause irritation. A small amount of blood or clear fluid is normal.

Dispose Safely

  • Place the used needle or syringe immediately into your sharps container — never recap
  • Never place sharps in regular trash or recycling
  • When your sharps container is ¾ full, seal and dispose per your local guidelines
  • Many pharmacies accept sharps container drop-offs for safe disposal

Tip: Check your state or city's guidelines for sharps disposal. Many areas have drop-off locations at pharmacies or health departments.

Tips for Reducing Discomfort

Most patients find GLP-1 injections cause minimal to no pain with proper technique. These tips can make the experience even more comfortable:

  • 1Let medication reach room temperature before injecting (30 min out of fridge)
  • 2Ensure the skin is fully dry after cleaning with the alcohol swab
  • 3Inject slowly and steadily — don't rush the plunger
  • 4Use a new, fresh needle for every injection — never reuse
  • 5Rotate injection sites consistently
  • 6Relax the muscle at the injection site — tensing up increases discomfort
  • 7Try the abdomen if you find other sites more sensitive

  • !Lumps or hard areas developing under the skin at injection sites
  • !Any uncertainty about your dose or injection technique

In case of a medical emergency — including severe allergic reaction, chest pain, or difficulty breathing — call 911 or go to the nearest emergency room immediately.
